Schedule or Change an Appointment

Current patients can schedule and change appointments through Simple Practice. If it is less than 48 hours before your appointment, please contact us via Simple Practice messenger.


Cancel an Appointment

  • Cancellations must be received 48 hours prior to the scheduled appointment to avoid fees. Failure to keep a scheduled appointment will result in a charge for the full fee of the scheduled appointment. Late cancellations will be charged at half the full fee. Please note that health insurance plans DO NOT pay for missed appointments; these charges will be entirely your responsibility.

Get a Refill

  • During your visit I will provide you with prescriptions for a supply of medication adequate to last until our next recommended appointment time.

  • If you run out of medication this is an indication that you are due for an appointment and refills will not be processed until you have an appointment scheduled.

  • If you require a refill between appointments please allow one week for processing of your request.

  • Please request a refill from your pharmacy. You must speak with the pharmacy staff to request controlled substance prescriptions.


Prepare for First Appointment

  • Parking is available in the spots marked “Visitor” which are near the two main entrances on the east and west sides of the building.

  • Please complete the online paperwork prior to the appointment

  • Please bring:

    • insurance card

    • current medications

    • a list of your previous medications

    • copies of any previous neuropsychological testing or diagnostics


Missed Appointments/Late Cancellations

Emergency/Crisis Situtations

  • Emergency Contact: If you are experiencing an emergency and/or need immediate assistance for your own or someone else’s safety, please call 911 or report to the nearest emergency room.

  • For the Crisis Line please call: 206.461.3222. Messages left on my voicemail are retrieved regularly and are returned within a week. However, I am only in the office three days a week so do not delay care waiting for a return call.


Extreme Weather

I follow the Seattle Public Schools extreme weather policy, if school is cancelled then we will make every effort to reschedule appointments. If school is running 2 hours late, I will start seeing patients at 11:30 am. If your appointment is before 11:30 am we will reschedule.
